By subscribing to the service, you agree to be bound by these terms and conditions.

  • To subscribe to the SMS service, you must opt in by providing your mobile number to Exemplar Health Benefits Administrator.
  • You acknowledge that message and data rates may apply. Exemplar Health Benefits Administrator is not responsible for any charges you incur.
  • You acknowledge that you may receive up to one or two messages per week.
  • You may opt out of the SMS service at any time by replying STOP to any message received.
  • Exemplar Health Benefits Administrator will promptly stop sending messages on receipt of an opt-out request.

Exemplar Health Benefits Administrator will not be liable for any loss, damage or inconvenience caused by use of the SMS service. You assume all risk and responsibility for its use.
