Prescription prior authorization — Elixir Solutions
Submit prescription prior authorization requests through CoverMyMeds, using the Elixir Solutions PA Request form.

Visit urmedwatch.com and select Precertification Request.

Call 502-499-6000 to request precertification.
Precertification — ServusCare
Visit the ServusCare Provider Portal and select Submit Request for Authorization.

Confirm the member is covered first. The member eligibility check returns coverage status, effective dates and plan codes from an NPI, a member ID and a date of birth — usually in under a second.
